The Student Achievement Data Collection Form is provided as a means of recording student activities and achievements, both academic and non-academic, which will be used as supporting data for accreditation and quality improvement of the Public Sector Accounting Diploma 4 Study Program.
To support the study program accreditation process and efforts to improve the quality of services and student development, the Public Sector Accounting Diploma 4 Study Program, Vocational Faculty, Universitas Sumatera Utara, provides a Student Achievements and Activities Data Collection Form.
This form is used as a means of collecting data on various student activities and achievements, both academic and non-academic, obtained at the local, regional, national, and international levels.
Students are expected to fill out this form completely and correctly if they have:
1. Academic achievements (contests, competitions, scientific works, awards, and the like)
2. Non-academic achievements (sports, arts, leadership, entrepreneurship, student ambassadors, and other student activities)
3. Participation in activities that represent the study program, faculty, or university
Students are expected to upload valid supporting evidence (certificates, charters, assignment letters, activity documentation, or other relevant documents) in accordance with the achievements or activities reported.
